The onset of a major, life-threatening disease is obviously and
absolutely traumatic not only for the unfortunate victim but also
for the whole family. Life Insurance may be of incalculable
assistance should the worst happen, but it is of no assistance at
all when help is really needed, when the problem is first diagnosed
and medical treatment is still a feasible option.
Various forms of Medical Expenses policies are available around the
world, but we can honestly say that the 'Dread Disease Insurance'
that we are proposing is by far and away the most simple and
attractive, for the insured.
Should an Insured Person be diagnosed during the period of Insurance
as suffering from a Dread Disease, symptoms (and/or the treatment)
of which were not present in such Insured Person at any time prior
to inception of the Policy or the date on which cover was granted to
such Insured Person and if the Insured Person survive the Diagnosis
for a period of 30 (thirty) successive days there after RELIANCE
INSURANCE LIMITED shall pay the Insured as Compensation" the Sum
Insured" set against such Insured Person's name in the Schedule.
Diagnosis means, Diagnosis by a registered medical practitioner,
supported by clinical radiological, histological and laboratory
evidence, acceptable to RELIANCE INSURANCE LIMITED.
Fundamentally, the insured selects a sum that he feels comfortable
with, both in terms of its premium cost and the cover offered, and
this becomes the amount paid to him-in full- immediately upon it
being confirmed that he has been diagnosed as suffering from one of
the defined 'Dread Disease'. The unique feature of this policy is
that this cash can then be used by him for whatever, and for that
matter wherever he chooses: for treatment locally, in India, in
Singapore or Bangkok, or even in London. Even, should the prognosis
be terminal, to cover the costs of visiting far away relatives, say
in the States, or bringing these relatives back to Bangladesh
absolutely whatever the Insured Person and his family wish to do
with it.

The moment it is established that the Insured is indeed suffering
from an Insured Dread Disease that had not manifested itself prior
to cover being issued, the policy pays. Pure and simple. |

OF FEATURES OVERSEAS MEDICLAIM INSURANCE (CFT) (CORPORATE FREQUENT
TRAVEL) |
|
|
|
Overseas Medical Insurance is available for Bangladeshi Nationals
going abroad for the purpose of Business or on Holiday. Annual
policy for CFT can be issued to the corporate client registered
under the companies Act or Government Service holders who are
regularly traveling overseas. This policy will cover for a total
period of 180 days overseas travel in the course of one year with an
inner limit of maximum 30 days per trip, |

a) typhoon, hurricane, volcanic
eruption, earthquake of other convulsion of nature.
b) loss or damage directly or indirectly caused by or arising
out of war, invasion act of foreign enemy, hostilities (whether
war be declared or not), civil war, rebellion, revolution,
insurrection, mutiny, riot, strike lock-out, civil commotion,
military or usurped power, a group of malicious person or
persons acting on behalf of or in connection with any political
organization, conspiracy, confiseations commandeering
requisition or destruction of or damage to property by order of
any government dejure of de facto or by any public authority,
nuclear reaction, nuclear radiation or radioactive
contamination.
c) any tests other than tests at a pressure not exceeding the
maximum pressure permitted by the inspecting authority.

PLANS TYPE: |

World-wide including
USA and CANADA and excluding the country of domicile.
SCOPE OF COVER:
Intended only for use of insured person in the event of a sudden and
unexpected Sickness or accident arising when the insured is staying
out side Bangladesh.
LIMIT OF LIABILITY: Maximum U.s. $100,000.00 and Minimum
$.10,000.00
COVERED EXPENSES:

1.
Expenses for physician service, Hospital, Physician and Medical
services and local emergency
medical transportation.
2. Up to US $ 225 per occurrence for dental services for
immediate relief of dental pain only.
3. Expenses for physician ordered emergency medical
evacuation, including medically appropriate
transportation and necessary medical care en route to
the nearest suitable hospital.
4. Expenses for medical evacuation including transportation
and medical care en route to a hospital on the
Insured Person's normal place of residence in the
foreign country where the insured person are staying.
5. If the insured person died outside the People's Republic of
Bangladesh the expenses for preparing the
air transportation of the remains for repatriation to the
People's Republic of Bangladesh or up to
equivalent amount for a local burial or cremation in
the country where death occurred. |

PERIOD OF INSURANCE:
4. The Insurance shall be valid from the first Day of insurance for
the number of days specified and ceases
on his return to Bangladesh wherever happens earlier.
5. The Insurance shall automatically be extended for a period of 45
days for treatment of illness and/ or
accident insured by the policy.
6. Extension of the period of Insurance is automatic for a period
not exceeding seven days, and without
extra charges if necessitated by delay of public transport
services beyond the control of the insured. |

BRIEF FEATURES OF
OVERSEAS MEDICLAIM INSURANCE EMPLOYMENT AND STUDIES |
Overseas Mediclaim Insurance for Employment and Studies is available
for Bangladeshi Nationals temporarily residing outside Bangladesh
having been posted by their Bangladeshi Employer to work in that
country or for the purpose of furthering their education or solely
engaged on research projects.
The Policy is divided into two categories: PLAN-C: Worldwide
excluding USA and Canada.
PLAN-D: Worldwide including USA and Canada.
SCOPE OF COVER:
Intended only for use of Insured Person in the event of a sudden and
unexpected Sickness or accident arising when the insured is staying
outside Bangladesh.
PAYMENT OF PREMIUM:
Premium is to be paid in US Dollars.
COVERED EXPENSES:
The reasonable and customary fees, charges for unavoidable necessary
medical services, supplies and treatments authorized by a Licensed
Physician and approved by the claims Administrator and incurred
outside Bangladesh.
SCHEDULE OF INDEMNIFIED EXPENSES AND BENEFITS.
1. Limited to US $ 75.000 anyone Insured Person and in all any
Period of Insurance.
2. Medical Accident and Sickness expenses as a result of a Covered
Injury or covered Sickness.
3. Coverage for mental, nervous and emotional disorders limited to
30 days continuous in hospital expenses only and will be limited to
only 50% of such expenses.
4. In the event that the Insured Person is to be evacuated to
Bangladesh the Insurers will pay Medical Accident and Sickness
Expenses in Bangladesh within the overall Limit of US$.75.000.
Expenses for Evacuation of Insured Person's spouse and Children
residing with him up to a limit of U.S. $5.000 anyone family.
5. In the event of the death of an Insured Person the actual expense
incurred for preparation and transportation to Bangladesh of the
remains of the Insured Person or Funeral expenses incurred in the
country of posting or study up to US$.8.000 in total will be borne.
6. When as a result of a covered injury or covered sickness an
insured Person is hospitalised and subsequently is to be medically
evacuated to Bangladesh Insurers will pay as Medical Emergency
Reunion Expenses up to US$.5.000 in all.
7. In the event the Insured person is unable to continue to complete
his course of studies due to covered injury or covered Sickness
first occurring in the country of study resulting in:
8. Death or (b) Loss of Entire Sight of either both Eyes or (c)
Permanent total Disablement and is Medically Evacuated.
9. The Insurers will pay a benefit to the Nominated sponsor who has
provided financial support to the Insured person at a rate ofUS$.750
Capital Sum for each month of study Completed during the period of
Insurance.
10. In the event that Insured Person is Unable to continue to
complete his course of Studies due to mental nervous or emotional
disorders the Benefit payable is limited to 25% of the amount due.
COMMENCEMENT OF INSURANCE:
The Insurance of an Insured Person will become effective from:
12.01 a.m. of the effective Date of the Insurance for which premium
has been paid.
the date the Insured Person departs from Bangladesh to travel to the
country of posting or study Provided that the Scheduled arrival in
the country of posting or study in no more than 48 hours later that
the insured person's departure from Bangladesh.
CLAIMS CONDITIONS:
In the event that medical services are required the Insured Person
shall wherever possible consult first with the campus, college,
school or employer's/sponsor's Doctor. In case of Hospitalisation
and out patient treatment in excess US$.500 the Insured Person, his
representative or the treating Licensed physician will Consult with
MERCURY INSURANCE SERVICES LTD. in UK, USA, CANADA (office address
are furnished in the IMPORTANT NOTES attached to the policy).

PLAN 'A': for countries other than USA and CANADA PLAN 'B' :
for countries including USA and CANADA.
SCOPE OF COVER: Intended only for use of insured person in
the event of a sudden and unexpected Sickness or accident arising
when the insured is staying out side Bangladesh.
LIMIT OF LIABILITY: PLAN 'A' U.S. $ 50,000.00 and PLAN 'B'
U.S. $ 100,000.00
COVERED EXPENSES:
1. Expenses for physician service, Hospital, Physician and Medical
services and local emergency medical transportation.
2. Up to US $ 225 per occurrence for dental services for immediate

PROCEDURE FOR OBTAINING CLAIM ASSISTANCE.
It is a condition precedent that when medical attention is
required VOYAGERS ASSISTANCE WORLDWIDE in UK (office address
are furnished in the IMPORTANT NOTES attached to the policy)
must be contacted by the Insured person, his representative or
the attending Physician / Hospital immediately. The Insured
person or his representative should quote to MERCURY as much
information concerning the illness or accident as is
available, including the name of the treating Doctor Name and
telephone number of the hospital the complete OMP Policy
number and its date of issue.

BRIEF OF THE FEATURES HEALTH PLAN INSURANCE |
|
|
THE HEALTH PLAN
INSURANCE is a group Medical Insurance Package that has been
adequately tailored to the requirements of the employees of the
corporate bodies. The Employers can take this Health Plan as a
welfare scheme for their salaried employees together with their
family members at a minimum cost of premium.
Brief of the Plan and Benefits of the Health Plan Insurance are
appended below.
LIMIT OF LIABILITY:
The total maximum amount of cover to each insured shall be limited
to Tk. 75, 000.00, tk.5O, 000.00 & Tk. 25,0001- respectively for the
three different plans known as SUPER PLAN, STANDARD PLAN & BASIC
PLAN.
SCOPE OF COVER:
ILLNESS, SICKNESS, SURGICAL OPERATION AND ANY TYPE OF ACCIDENT.
MAIN FEATURES:
1. An employee shall be eligible for membership of the plan on the
date on which he completes at least 90 days of continuous active
service with the employer and anyone within the age limit of 18 to
60 years may obtain this insurance policy.
2. An employee can be a member at any time after the effective date
of this contract paying pro rata basis premium.
3. A member under this plan would be under 60 years (and shall cease
to be a member the date on which the member retires or attains the
age of 60 years, which ever is earlier).
4. The employer shall be entitled to receive "No claim discount" in
respect of a member that there shall not have been any claim through
out the year.
5. Period of Insurance is one year effective from the date of
commencement and this contract is renewable. |

GENERAL CONDITIONS
1. Policy liability for each Confinement shall be limited to a
maximum of twenty-one days. Successive periods of hospital
confinement within 90 days due to the same disability are considered
as single period. However, in no event shall the benefit exceed the
limit set forth in the Schedule of this contract. On the other hand,
confinement shall mean an admission to a Hospital within the
territory of Bangladesh for a period of at least twenty-four
consecutive hours.
2. There is a PRE-EXISTING CONDITION in the policy, i.e. any illness
or disability of which symptom') were evident or for which he
(Insured Person) received medical advice or treatment within twelve
months immediately prior to the date of his membership of the plan
is not covered under this Insurance.
3. Any charges / cost for Foods or Food supplements, Cosmetics of
any nature, Water purifier are excluded.
4. Maternal benefits for pregnancy or childbirth are covered only
after nine months from the commencement date of this contract etc. |
